Bengaluru: Doctors at SPARSH Hospital, Yeshwanthpur, successfully managed a rare and complex high-risk pregnancy in a 21-year-old woman with a permanent cardiac pacemaker, enabling the safe delivery of a healthy baby boy through a planned Caesarean section.
The case involved multiple clinical challenges, including congenital heart disease, a permanent pacemaker, and significant placental abnormalities that increased the risk of severe bleeding during childbirth.
The first-time mother had undergone corrective surgery for congenital heart disease during childhood and subsequently received a permanent pacemaker for a cardiac rhythm disorder. During her pregnancy, doctors identified large placental lacunae, placing her at an elevated risk of postpartum haemorrhage (PPH). Given the combination of cardiac and obstetric risks, the pregnancy required careful monitoring and extensive preparation for delivery.
A multidisciplinary team comprising specialists from Obstetrics & Gynaecology, Cardiology, Anaesthesia, Neonatology and Cardiac Technical Services jointly assessed the patient and developed a detailed delivery plan. Following comprehensive cardiac evaluation and fitness assessment, the team proceeded with an elective Lower Segment Caesarean Section (LSCS) under spinal anaesthesia.
To ensure continuous cardiac support during the procedure, a dedicated pacemaker technician remained present inside the operating theatre throughout the surgery. During the Caesarean section, the patient developed postpartum haemorrhage associated with the placental abnormalities. The obstetric team promptly identified the bleeding and initiated advanced medical and surgical measures, successfully controlling the haemorrhage while maintaining the patient’s stability.
The procedure concluded successfully, with the birth of a healthy male baby weighing 2.9 kg. Both mother and baby remained stable during the perioperative period and the mother subsequently made an uneventful recovery before being discharged in good health with her newborn.
